How to Make a Referral
To make a referral, contact the appropriate treatment location or fax referral information directly to our team. Our staff will coordinate next steps with the referring party as permitted by applicable consent and privacy requirements.

- Confirm the patient's consent to share clinical information
- Call the receiving location during clinic hours, or fax referral information
- Include relevant clinical history and current medications if consent allows
- Our team contacts the patient and schedules an assessment
- Coordination of care continues as permitted by written consent
Disclosure of substance use disorder treatment records is governed by federal confidentiality rules and requires appropriate patient consent.

Clinical services
What Referred Patients Receive
Both locations provide the same core clinical services under the same organizational standards. Insurance is accepted, and self-pay options are available.
Medication for Opioid Use Disorder (MOUD)
Medication may be used as part of an individualized treatment plan and is prescribed and monitored by qualified clinical professionals.
Individual Counseling
One-on-one counseling helps patients address recovery goals, challenges, behaviors, relationships, and long-term stability.
Group Counseling
Structured group services provide education, support, skill building, and connection with others working toward recovery.
Assessment & Individualized Treatment Planning
Care begins with a clinical assessment and continues through an individualized plan based on clinical needs and recovery goals.
Medical & Clinical Services
Patients receive ongoing medical and clinical monitoring by our medical and nursing team throughout treatment.
Clinical Monitoring & Drug Screening
Clinical monitoring, including drug screening, supports patient safety, accountability, and adjustments to individualized care.
Recovery & Community Support
Treatment may connect individuals to recovery-support resources, educational opportunities, community programs, and supportive services when available.
